Apply for a practical driving test application form

You can use the practical driving test postal application form to book by post if you can't use the online or telephone booking services.

How to use this service

Download the application form you need from the link below and fill it in.

You can pay for your test by cheque or postal order, but cash payments aren't accepted. You should make your cheque or postal order payable to 'Driving Standards Agency'.

Send your completed application form with payment to: Driving Standards Agency, PO Box 280, Newcastle-Upon-Tyne, NE99 1FP.

Tests for cars, mopeds and motorcycles, taxis and approved driving instructors (ADIs)

You can use this form to book the following types of practical tests:

  • car
  • moped and motorcycle
  • taxi
  • ADI

Tests for lorries, buses, coaches and cars with large trailers

You can use this form to book the following types of practical tests:

  • large goods vehicle (LGV)
  • passenger carrying vehicle (PCV)
  • car with a large trailer
