If you are applying for or currently hold a car or motorcycle (group one) driving licence

Download the appropriate information leaflet or medical questionnaire to tell the Driver and Vehicle Licensing Agency (DVLA) about your medical condition or disability. The questionnaire includes a consent form that must be signed, allowing DVLA to make medical enquiries into your fitness to drive.

Where an information leaflet relating to your condition is available, please download it, read thoroughly and keep for future reference. Should the leaflet indicate that medical enquiries need to be made please download and complete the appropriate medical questionnaire as indicated.

Diabetes

For the following condition:

  • diabetes treated by tablets, diet or both

For the following condition:

  • diabetes controlled by insulin

Heart conditions

For the following conditions:

  • heart attack (acture coronary syndrome to include myocardial infarction)
  • coronary artery by-pass surgery (CABG)
  • coronary angioplasty (also known as percutaneous coronary intervention)
  • heart valve disease/surgery
  • angina

For the following conditions:

  • implanted pacemaker
  • disturbance of heart rhythm significant to cause dizziness, collapse or loss of consciousness
  • any other heart condition

For the following condition:

  • defibrillator (ICD) implanted

Neurological conditions

For the following conditions:

  • stroke
  • mini stroke
  • cerebral thrombosis
  • amaurosis fugax

For the following conditions:

  • brain surgery, brain tumour, severe head injury
  • serious memory problems or episodes of confusion

For the following condition:

  • chronic neurological conditions (eg multiple sclerosis, motor neurone disease)

For the following condition:

  • epilepsy, fits or blackouts

For the following condition:

  • severe and recurrent disabling giddiness

For the following condition:

  • parkinson's disease

Sleep disorders

For the following conditions:

  • sleep disorders or conditions which cause excessive daytime or awake time sleepiness
  • sleep apnoea syndrome
  • narcolepsy or cataplexy
  • any other condition which causes excessive daytime or awake time sleepiness

Vision problems

For the following conditions:

  • any visual condition which affects both eyes (not including short or long sight or colour blindness)
  • any visual condition which affects remaining eye, if sight in one eye only (not including short or long sight or colour blindness)

Alcohol and drugs

If you’re disqualified for some alcohol related offences, DVLA will make medical enquiries before your driving licence can be renewed.

These offences are:

  • disqualified with an alcohol level of over: 200mg in 100ml of blood; or 87.5mg in 100ml of breath; or 267.5mg in 100ml of urine
  • disqualified twice in 10 years for certain alcohol related offences
  • disqualified for failing or refusing to give a specimen (blood, breath or urine) for testing

Psychiatric conditions

For the following conditions:

  • any mental ill-health condition (including depression)
  • any psychiatric illness requiring hospital admission

For the following conditions:

  • alzheimer’s, cognitive impairment, dementia, memory problems

Spinal injuries

For the following conditions:

  • severe spinal injuries
  • paraplegia
  • tetraplegia

Limb problems

For the following conditions:

  • amputations
  • persistent limb problems
